Application Forms

A student will not be considered for admission until the school has received a complete a new student information form, which includes the following:

All new applicants need

• Completed new student information form
• Birth Certificate
• Current immunizations
• Baptismal Certificate (Catholics)

New students applying for Grades 1-8 need

• Copies of Report Cards for last 2 years.
• Copies of any Standardized Tests for the last 2 years.

New students must be admitted before completing a tuition contract.
